Follow up visit/ medication management – $ 200 per session
Psychotherapy (30 minutes) – $150

Any forms requiring provider opinion – $50
Any forms more than 4 pages and requiring physician opinion- $100
Emotional Support Animal letter and assessment- $100
Any letter requesting medical opinion – $100
